The mystery surrounding Hayden Panettiere’s death has an answer. The Greenville County Coroner’s Office has ruled her death an accidental overdose — and the toxicology report reveals exactly what was in her system.

When we first reported on Panettiere’s death back in August, the cause was still unknown and the DEA had just joined the investigation. Over a month later, the results are in.

What the Coroner Found

The Greenville County Coroner’s Office confirmed this week that Panettiere’s cause of death was ruled the toxic effects of five substances: fentanyl, its chemical byproduct 4-ANPP, the anti-anxiety medication alprazolam (Xanax), the muscle relaxer methocarbamol, and quetiapine, a drug used to treat serious mental health conditions. Officials said there were no signs of trauma.

Quick Facts
  • Panettiere, 36, was found unresponsive on August 16 at Judson Mill Lofts in Greenville, South Carolina
  • Cause of death: accidental overdose involving fentanyl, 4-ANPP, alprazolam, methocarbamol, and quetiapine
  • Her on-again, off-again boyfriend Brian Hickerson and his brother Zach were at the scene
  • The DEA is still investigating whether anyone could be criminally responsible for providing the drugs
  • Panettiere had long been open about her struggles with addiction, dating back to her teenage years

Filling in the Gaps From Our Original Report

When Panettiere was first found unresponsive on August 16, authorities said only that some reports had mentioned a possible overdose, while stressing that nothing was officially confirmed. The DEA joined the Greenville Police Department’s investigation days later, and her representative said at the time there was nothing more to share while the case remained open.

Now the toxicology results close that particular loop. Brian Hickerson and his brother Zach were present when Panettiere was found, and a police report obtained by CBS News noted that Hickerson showed officers a bag of medication she’d been taking. Zach later told TMZ that Brian had given her Narcan, a medication that reverses opioid overdoses, before first responders arrived.

What hasn’t closed is the question of accountability. Even with the death ruled accidental, authorities — including the DEA — are still investigating whether anyone could face charges for supplying the drugs.

A Life She Had Only Recently Opened Up About

Panettiere had been candid about her struggles well before her death. She’s said she became addicted to alcohol and opioids as a teenager, and she revisited that history in detail in her 2026 memoir, “This Is Me: A Reckoning,” alongside experiences with physical abuse and postpartum depression.

Her relationship with Hickerson had drawn scrutiny for years. He was charged in 2020 with multiple counts of felony abuse against Panettiere and pleaded no contest to two counts of injuring a spouse or partner, serving jail time and completing probation, anger management and Alcoholics Anonymous requirements.

Panettiere’s family had already faced devastating loss: her younger brother and fellow actor, Jansen Panettiere, died in 2023 at 28 from a heart condition. She later said losing him took a serious toll on her mentally and physically.

Why This Combination Matters

Forensic experts note that the mix found in Panettiere’s system is a familiar and dangerous one. Fentanyl alone can be lethal, and layering it with drugs like alprazolam and methocarbamol compounds the risk significantly — a combination pathologists say they see far too often in overdose cases.

Her death adds to a grim national pattern: fentanyl and other synthetic opioids remain involved in more than half of all U.S. overdose deaths, even as overall numbers have started to decline.

The “why” behind Hayden Panettiere’s death finally has an answer. The “who,” if anyone, still doesn’t.
